How to become a Day1 Health member

  • Complete our online application form by clicking here
Submitting your application form

  • Fill in the application form in black ink, using block letters. Please print clearly.
  • Answer all medical questions truthfully and include as much details as possible.
  • Please make sure the main applicant signs and dates any changes.
  • Fax the completed and signed form to 086 690 4818 or email it to admin@day1.co.za.
  • Please attach a copy of each applicant’s identity document. We also accept valid passports and birth certificates for children.
  • Make sure you indicate your chosen network doctor from our network providers list available in the members area on the application form.
Once you send us your application form, here is what will happen

  • If any details are missing or if we need more information for underwriting purposes, we will contact you.
  • We will activate your membership and send you a confirmation sms within 24 hours containing your membership number and important contact information.
  • You will receive your Day1 Health welcome pack after your first successful debit order (once off activation fee charged).
  • Your welcome pack contains your Day1 Health membership card, welcome letter, ER24 car sticker, policy schedule (applicable to hospital policies only), product guide, and endorsements if applicable.
  • Please read your documentation carefully and keep safe for your records.
